Please confirm these 4 legal requirements before the notary arrives:
The patient must have an unexpired TX Driver's License, State ID, or US Passport. If missing or expired, contact Robert immediately to arrange Texas Credible Witnesses.
The patient must be awake, alert, coherent, and fully understand the document they are signing. We cannot legally notarize if the signer is unresponsive or heavily sedated.
All blanks on the document must be filled in ahead of time. However, the document must remain completely UNSIGNED until the notary is standing at the bedside.
If the document requires witnesses, hospital and nursing home staff legally cannot sign. Please have independent neighbors, friends, or non-relatives present.
